Adecco is currently seeking a compassionate and experienced Nurse Case Manager to join a dynamic healthcare team located in Toronto, ON. This is a Temporary position offering $45 per hour. The Nurse Case Manager will provide specialized nursing care and case management services for clients with psychiatric and dual diagnosis issues. As a member of the team, you will ensure that clients receive quality care, advocate for their needs, and provide consultation and education to clients, families, and healthcare providers.

Responsibilities:
Serve as the primary case manager for up to 10 clients, ensuring the implementation of personalized care plans.
Assess client needs and revise care plans based on ongoing evaluations.
Dispense and monitor medications, providing clients with education on healthy lifestyles.
Provide counseling to clients to manage their symptoms and daily stressors, including assistance with activities of daily living (ADLs).
Administer crisis intervention support as needed and advocate for additional services or resources for clients.
Work collaboratively with other healthcare professionals, including hospitals and community service providers, to promote continuity of care.
Facilitate the development of new services, policies, and procedures within the organization.
Maintain detailed client records and ensure compliance with confidentiality regulations, including PHIPA.
Attend staff meetings, participate in educational opportunities, and stay updated on healthcare best practices.
Contribute to the development and delivery of education to staff, community service providers, and other stakeholders.
Participate in research, evaluation activities, and committees to improve service delivery.
Ensure that all nursing practices comply with the College of Nurses of Ontario's standards and ethical guidelines.
Follow health and safety regulations as outlined by our client’s Joint Health & Safety Committee.
Requirements:
Must be legally eligible to work and reside in Canada.
A graduate of an approved School of Nursing and current Certificate of Competence as a Registered Nurse with the College of Nurses of Ontario.
At least 3 years of experience in psychiatric or community psychiatric nursing.
Knowledge of the nursing process, including assessment, planning, implementation, and evaluation.
Strong interpersonal skills, with the ability to resolve conflicts, negotiate, and manage people effectively.
Proficient in MS Office and other software applications, with the ability to learn new systems quickly.
A valid Ontario driver’s license and access to a reliable vehicle are required.
Experience with substance abuse, dual disorders, or legal issues would be an asset.
Bilingualism (second language) and experience working with diverse populations is an advantage.
Ability to work independently, as well as part of a collaborative team.
