Adecco is currently seeking a compassionate and skilled Case Manager - Crisis Outreach Services for Seniors to join a dedicated team supporting seniors facing mental health, substance use, and other complex health challenges. The ideal candidate will be experienced in crisis support and intervention, with a focus on helping seniors through recovery and harm reduction approaches. This position requires an individual who can manage crisis situations, assess client needs, and collaborate with a variety of community resources to develop and implement care plans for seniors in need of immediate assistance.

Responsibilities:
Handle crisis calls initiated by clients, family members, friends, health care providers, and community agencies. Assess immediate needs and determine next steps.
Conduct client needs and risk assessments to develop personalized care plans that include both short- and long-term objectives.
Provide short-term crisis stabilization case management services, including assessments, intensive case management, long-term planning, and follow-up services as appropriate.
Refer clients and their caregivers to a range of community resources, including primary care, financial aid, food programs (e.g., Meals on Wheels), assisted devices, and social/recreational programs.
Participate actively in developing, implementing, and evaluating the Crisis Outreach Service through strategic planning, meetings, and research initiatives.
Willingness to work flexible hours, including evenings, weekends, and statutory holidays as required.
Participate in an on-call rotation to provide crisis support when needed.
Requirements:
Must be legally eligible to work and reside in Canada.
University degree or Community College Diploma in Social Services or a related field. A BSW or MSW is an asset.
A minimum of 3 years of related experience in crisis support and intervention, particularly in a senior care setting.
Extensive knowledge of senior-specific issues, including mental health, substance use, and other complex health challenges.
Experience with harm reduction and recovery-oriented approaches is essential.
Strong verbal and written communication skills.
Demonstrated experience in case management and referring clients to appropriate services.
Ability to work independently, but also collaboratively as part of a multi-disciplinary team.
Proficient in Microsoft Office, Client Information Systems, and web browsing.
Fluency in a second language and access to a personal vehicle (valid driver’s license required) would be an asset.
Must undergo a criminal background check.
