Adecco is currently seeking a compassionate and experienced Case Manager to join a dynamic team providing support to individuals facing mental health and addiction challenges in the Toronto area. This is a temporary, contract position offering an hourly wage of $30 to $35. The Case Manager will work with clients, their families, healthcare providers, and community agencies to deliver comprehensive care, assessments, and support services. The ideal candidate will have a background in case management and a strong understanding of mental health, addiction, and recovery.

Responsibilities:
Screening and responding to calls initiated by clients, family members, healthcare providers, and community agencies.
Conducting client needs and risk assessments to create personalized care plans with both short-term and long-term objectives.
Providing short-term crisis stabilization case management services, including assessment, intensive case management, long-term planning, and follow-up as appropriate.
Referring and connecting clients and caregivers to a range of community services, such as primary care, financial aid, assisted devices, home safety assessments, food banks, and social programs.
Actively participating in the development, implementation, and evaluation of the Crisis Outreach Service, through meetings, strategic planning, and research initiatives.
Collaborating with a multi-disciplinary team, including internal and external stakeholders, to ensure clients receive the best possible care.
Requirements:
Must be legally eligible to work and reside in Canada.
A University degree and/or a Community College Diploma in Social Services or a related field (BSW or MSW preferred).
A minimum of three years of relevant case management experience.
Experience supporting the community with mental health, substance use, and other complex health issues, from a recovery and harm reduction approach.
Extensive knowledge of senior issues and services.
Experience working with clients with age-related mental and/or physical health issues.
Excellent verbal and written communication skills.
Proven ability to assess client needs and link them to appropriate resources.
Strong computer skills (Microsoft Office, Client Information Systems, Web browsing).
Fluency in a second language is an asset.
A valid driver’s license and access to a personal vehicle is required.
Willingness to work flexible hours, including evenings, weekends, and statutory holidays, and participate in an on-call rotation.
A criminal background check will be required.
Proof of at least two COVID-19 vaccinations is required.
